Job summary

Are you interested in working for an exciting proactive team? Enfield Crisis Team is looking for 3 full-time band 6, nurse practitioner based in Chase Farm Hospital. This is an exciting opportunity for an independent and self-sufficient practitioner. Experience working with adult who have mental health difficulties, previous experience of working in multi-disciplinary team, and an understanding of crisis intervention is required. The individual should be dynamic, pro-active and forward thinking and have a clear focus on patient and carer experience.

Main duties of the job

To provide a high standard of assessment and treatment for people referred to the CRHT, whilst complying with the philosophy, CRHT operational policies in addition to the Trust's policies, practice and procedures.

To conduct accurate risk assessments and management plans, involving the service user, carer, and any other individuals relevant to the care of the service user, and to ensure that this is well documented.

To ensure that all team members complete accurate risk assessments andmanagement plans.

To ensure effective liaison with bed management services whilst ensuring the safety of service users is maintained in the community.

To assess service users meeting defined referral criteria as per local CRHT Service policies.

To assess, plan, implement and evaluate programmes of care for serviceusers with acute mental health problems under the care of the CRHTservice.

To participate in the Care Programme Approach and ensure that relevantCPA documentation and CRHT Service responsibilities under current policies are maintained

To promote the supervision/ administration of medication and monitoring therapeutic and adverse effects of prescribed medication as per Trust and CRHT Service policies.

To ensure that all relevant information regarding service user's care iscommunicated to the appropriate persons both within and outside the team with supporting high quality written information.

  • Mentorship qualification or equivalent

PROFESSIONAL EXPERIENCE

Essential

  • At least 2 years post-registration experience at band 5 level.
  • Experience of acute inpatients and/or community areas, working with people going through a mental health crisis.
  • Evidence of continuing professional development
  • Experience of working autonomously in a community setting
  • Experience of working in a crisis resolution/home treatment service
