Job summary
This is a permanent role created to help us deliver our plans to transform our community mental health services, as part of the NHS Long Term Plan and working within our Primary Care Networks. The MH ACP role will work within GP practices supporting people who identify with mental illness.
Our Mental Health transformation plans are supported by £25m in funding and will mean the roll-out over the next three years of multi-disciplinary teams of experts working within the new Primary Care Networks and offering wraparound, holistic care to our service users and wider community. Our aim is to support the whole person as they live with, or recover from, mental illness and to offer them support with other challenges they may face, such as debt, relationship problems, housing, education or training. We will also focus resources on reaching those who currently face obstacles in getting the help they need.
The Mental Health Advanced Clinical Practitioner (MHACP) will use highly specialist knowledge and skills to provide healthcare autonomously to people within their sphere of practice, acting as the Mental Health expert. The MH ACP is accountable for independent clinical assessment, diagnosis and treatment of people. In addition to this, the MH ACP is also responsible for appropriately referring people to relevant specialties for any necessary investigations.
Main duties of the job
- Act as the first point of contact exercising Mental Health clinical and professional autonomy.
- This involves critical judgement and expertise in providing clinical assessment and interpretation of clinical findings to ensure that the most appropriate care is delivered to patients referred with mental health care needs.
- Prioritise health care needs and intervene appropriately to assist the patient in complex or urgent care situations and escalate as needed.Independently prescribe (if within scope of professional practice) and review medications for therapeutic effectiveness, appropriate to patient need and in accordance with evidence-based practice and national and local policy and protocols.
- Work in partnership with patients, Doctors, other health and social care professionals, carers and relatives and the voluntary services sector in the planning and delivery of care and the safe transfers of care between organisations
- Demonstrate critical thinking and analytical skills incorporating critical reflection.in the clinical decision-making process.
- Analyse and interpret clinical history, presenting symptoms, physical and psychological findings, and diagnostic information along with physical examination to reach a differential diagnosis.
- Recognise the limits of their scope of professional practice and know when to refer to the patient's GP.
