Job summary

The Royal Orthopaedic Hospital NHS Foundation Trust is the largest provider of elective orthopaedic surgery in the UK and provides routine and specialist orthopaedic services. It is also the leading specialist centre for bone and soft tissue cancers and provides a regional spinal service.

This is an exciting opportunity to be part of the team in driving forward medicines management and clinical pharmacy projects within the Trust. This will involve conducting multi-disciplinary ward rounds, contributing to the development of new medicines management and clinical guidelines, and providing support to deliver on national and regional quality indicators.

We are looking for a highly motivated individual, with good clinical skills who is keen and effective at implementing changes. We are looking for a Band 7 clinical pharmacist who has completed their clinical diploma or can work towards completing it.

The successful candidate will help develop the pharmacy service in line with the aspirations of the pharmacy department, the Division and the Trust.

Main duties of the job

The post holder will be working as part of a team of clinical pharmacists to work on wards and in theatres to deliver high quality evidence-based pharmaceutical services.

The post holderwill be expected to help develop and implement:

  • processes of pharmacy service delivery and clinical governance
  • The Medicine Code - including undertaking audits to fulfil governance and contractual requirements.
  • Integration of technology into the delivery of clinical pharmacy services to the ward teams

One of the key objectives of the role will be to help deliver the Discharge Medicines Service (DMS) initiative at the Royal Orthopaedic Hospital NHS Foundation Trust. This shall help to establish partnerships with external organisations to facilitate better communication between colleagues in primary care and secondary care to reduce the risk of harm and improve patient experience.

The position will also involve supporting the delivery of action plans relating to safety alerts and the teaching of Medicines Management to medical, nursing, pharmacy and other health care workers in the Trust by working with senior pharmacists. The post holder will also have a clinical commitment which includes on-calls and weekend cover.

Job description

Job responsibilities

  1. To be a clinical pharmacist to provide high quality pharmaceutical care to patients and their carers in the Trust by undertaking ward rounds and attending MDTs for surgical patients
  2. To work closely with medicines management technicians to ensure the delivery of an integrated pharmacy service across the hospital
  3. To develop expertise in using the Trust EPMA system and helping other users to improve medication reconciliation, prescribing, administration and discharge planning
  4. To demonstrate and utilise clinical pharmacy knowledge through personal development.
  5. To work collaboratively within the multidisciplinary team to deliver excellent care to all patients through specialist and consultant ward rounds, clinics and MDT meetings
  6. To act as a role model in the field of clinical pharmacy and provide a consistently high level of pharmaceutical service in-line with Trust objectives and values
  7. Audit, monitor and develop medicines optimisation in line with pharmacy strategy aims and objectives

RESPONSIBILITIES

Clinical Pharmacy

To work with the other pharmacists in the provision of a high-quality clinical pharmacy service to wards and theatres in the Trust including the dispensary when required. This includes core clinical pharmacy activities comprising of:

  • Assessing prescription charts on the electronic prescribing system (PICS) in order to ensure safe rational and effective prescribing, establishing drug histories, enabling use of patients own medicines and patient self administration, and counselling to improve medication usage and patient understanding during in-patient stay or at discharge
  • Interpreting and requesting clinical tests to monitor and make recommendations to improve effectiveness of drug therapy
  • Making recommendations about medications to improve patient outcomes and to encourage adherence to hospital formulary and national guidelines
  • Providing medicines for individual patients during their stay in hospital, at discharge and also for out-patient visits
  • Being accountable on the ward or theatre assigned for adherence to all pharmacy policies and procedures relating to medicines management
  • To provide drug knowledge to proactively influence safe and effective drug therapy in conjunction with and by liaison with other clinical staff. This may involve reviewing published literature and giving advice on experimental drugs or those where limited evidence exists
  • To attend consultant ward rounds, clinics and multidisciplinary team meetings (including directorate and/or risk meetings) as appropriate
  • To provide advice and guidance on all issues relating to prescribing on wards or theatres served in line with national and local guidelines
  • To help in the development and implementation of systems of safe medication practice that lead to reduction in operational and clinical risk in respect of medications, and in particular intravenous therapy
  • To assist in the development, implementation and auditing of prescribing guidelines and protocols for all aspects of practice in ROH. This will be in conjunction with relevant medical, pharmacy and/or nursing or theatre staff
  • To undertake research, audits and development activities in Medicines Management, Prescribing and Pharmacy practice, to ensure continual progression of services and ensure that nationally agreed targets are reached
  • To contribute to the submission of business cases to secure funding for new drug developments, and prepare new product evaluations for the Drugs and Therapeutics Committee
  • To support the Chief Pharmacist and Deputy Chief Pharmacist in all aspects of Formulary Management and management and analysis of the drugs budget
  • To assist appropriate non-medical staff in the production and checking of Patient Group Directions (PGDs) as requested
  • To support the lead/ link directorate and specialist pharmacists by attending appropriate directorate/speciality meetings and analysing economic drug use data, monitoring drug usage and providing data with interpretation and ideas of cost saving initiatives as requested
  • To help in the investigation of incidents and audits that the senior pharmacists may request.

Education & Training

  • To participate in the requirements of the GPhC for Continuing Professional Development
  • To supervise necessary training of Medical, Nursing and other clinical staff with respect to relevant areas of Clinical Practice/Medicines Management
  • To participate in the development and training of the Ward Technicians working in the specific area and contributing to the development of this service

General

  • To help in the development of performance indicators that clearly demonstrate the contribution that this post and pharmacy is making to the care of patients in the Trust
  • To participate in the development of any departmental SOPs that are required
  • To deal with face to face complaints on service from patients, prescribers and other healthcare personnel and investigate them as required, following Trust Procedures
