Job summary
The post holder will be a registered nurse (children's or learning disability) using their knowledge and experience in the care and management of children with palliative care needs. They will deliver safe, personal, effective and compassionate care to children, young people and their families, ensuring empathy and quality. They will work alongside and be supported by other band 7 Kentown nurses working across Greater Manchester to help develop children's community palliative and end of life care, promoting a holistic approach to 24/7 care.
Please note: whilst the role is advertised as a Band 7 Kentown Children's Palliative Care Community Nurse Specialist, there may be an opportunity to be appointed as a Band 6 Kentown Children's Palliative Care Community Nurse. This will provide the opportunity for further study and learning supported by partner agencies to complete competencies to allow the post holder to move into the band 7 specialist role.
Main duties of the job
To develop skills as a Kentown Children's Community Palliative Care Specialist Nurse supporting and advising children, young people and their families facing the end of their child's life, at the time of death and following death.
To develop skills in the assessment, planning and development of individualised plans of care to meet the needs of the child, young person and their families so that appropriate care and support is established and maintained as they move between hospital and community settings.
To develop skills in the coordination and mobilisation of services, complex discharge planning to support the patient and family during palliation and bereavement and signposting them to the most appropriate professional to act as key worker.
To develop skills in advance care planning by networking with colleagues and undertaking training to ensure that all children and families are offered an advance care plan.
Give advice to families and carers on practical issues following death of a child and signpost to appropriate help and support.
