Job summary

The Community Learning Disabilities Team are looking to recruit a Support Worker to join our friendly, forward thinking, multidisciplinary team.

The post holder will work within the Community Learning Disability Team to provide care and treatment to patients in the community setting, including the patients' homes.

To be eligible for this post you must meet these criteria:

  • As the role involves visiting patient homes & clinics across the Trust every day, postholders required to have access to a car with suitable business insurance. We will carry out a screening as part of our shortlisting process and only those meeting this requirement will be invited to interview.
  • Postholders must have minimum NVQ Level 2 in Health & Social Care (from an RQF accredited provider) or Care Certificate (obtained through observation at work, not through the short online modules).

Entry level salary for this post does not meet the minimum salary threshold required for sponsorship under the Skilled Worker visa route. Applicants who may require sponsorship are advised to check the current visa requirements on the GOV.UK website before applying.

This is a permanent post working full time (37.5 hours) across Monday to Friday. Service hours are mostly 0900-1700 but flexibility is required to meet patient need.

Main duties of the job

  • To be a member of a Multi-disciplinary team focusing on the needs, intervention and treatment of individuals who have a learning disability.
  • The post holder will focus upon ill health prevention and health promotion, delivering treatment and behavioural management plans for individuals who have highly complex and specialist needs.
  • Key working relationships with all members of the multi-professional team, including nursing, therapists, social workers, and other agencies
  • To carry out assigned tasks involving direct care in support of a Registered Nurse bysupporting individuals in the achievement and / or maintenance of their health and wellbeing this can include health promotion for client group.
  • This may also involved when competencies achieved: blood pressures, blood samples, finger prick technique, urinalysis, bowel care, nutritional support, observations, specimen collection, prevention of pressure ulcers, moving and handling, weight - hoist, wheel chair, height, urinalysis, oxygen statistics, respirations, pulse, waist measurements,
  • Promote personal care skills of the individual
  • Carrying out behavioural and clinical observations as instructed by the registered nurse
  • Support people with learning disabilities to access generic screening services

Planning and Organisational Duties

To operate within the organisations clinical governance framework

Ensure confidentiality is maintained at all times.

To adhere to trust policies and procedures.

Ensure clear understanding of your own role and responsibilities within sphere of competencies, by influencing the development of own knowledge ideas and work practice.

Attend and participate to client focused person centred and multi-disciplinary team meetings.

Work in line with the organisational lone worker policy

Maintain own skills attend mandatory training.

Implement health promotion/ education to 1-1 or a small group as appropriate e.g. smoking healthy living etc

Assist in the health promotion and education along with the registered professionals.

Communications and Key Working Relationships

Work in collaboration with the MDT under the supervision of the Registered Nurse

Work independently under a care plan from the registered nurse

Communicate findings to the Registered Nurse to assist with assessment and implementation of care to the person with a learning disability.

Assist with the identification of need for ongoing referrals and have a knowledge of services available to support with health promotion, wellbeing of the person.

Assist the Registered Nurse in establishing and maintaining an effective communication system.

To work and communicate appropriately in situations that may be sensitive or contentious.

To undertake project work under the supervision of the Registered Nurse.

To maintain accurate patient records in accordance with trust guidance and data protection standards.

Responsibility for Finance

Identify and report disrepair of equipment.

Identify own need for personal development, attend mandatory training.

Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions.
