Job summary
This vacancy is for a permanent part time (36 hours per week) Community Matron. Working pattern is 3 or 4 shifts between Monday and Friday in the Leigh Community Matron team.
The ability to be able to travel between different Trust sites is essential.
All candidates will be required to evidence their original certificates for qualifications to the interview panel. An Enhanced DBS with check against the Adults Barred list will be obtained for the successful candidate during pre-employment checks.
Main duties of the job
The post holder will be a highly skilled practitioner, with the ability to work autonomously, with experience in clinical examination/diagnostics and long term conditions management.Problem solving skills and excellent communication skills are essential.
As part of the role you will co-ordinate the identification of patients with chronic long term conditions, who are at high risk of admission to hospital and who will most benefit from case management.

- Work as an autonomous clinician in a defined area to develop and provide expert clinical advice, support and care to a defined group of patients.
- Develop and use advanced clinical skills to analyse and interpret history of illness, presenting symptoms and physical findings to enable diagnosis, planning and treatment of patients.
- Empower patients and their families through appropriate support and education to manage their long term illness or disability.
- Develop patient self-management plans.
- Facilitate the integration of patients goals for health and social care plans and future health needs.
- Develop working relationships with primary and secondary care clinicians to ensure patients care is of a high quality and appropriately delivered.
- Establish partnerships and networks with social services, voluntary sector and other agencies to ensure patients access all resources available, working closely with social care teams.
- Provide and co-ordinate the interface between acute, primary, community and social care settings in order to reduce unnecessary attendances/admissions to hospital and facilitate early discharge if possible
- Be part of an integrated community team attending regular team meetings to plan and organise patient management plans.
- Collect, collate, evaluate and report clinical information, maintain accurate patient records related to assessment and care planning.
- Develop clinical protocols and guidelines within own area of practice to ensure quality of care at all times.
- Review medication, discuss side effects and where appropriate prescribe relevant medication or liaise with medical staff.
- Regularly evaluate practice to ensure continuous improvement.
