Job summary
This is a permanent full time post (37.5 hours) to be worked across Monday to Sunday with shifts between 0800-1800. The post is based at Wigan Life Centre South.
You must have NHS clinical experience and either Level 3 in Health & Social Care or else relevant equivalent experience in a similar healthcare field.
An Enhanced DBS with Adults' Barred List check will be obtained during pre-employment checks.
Main duties of the job
- To work along side our social care colleagues to plan and facilitate safe and timely discharge of patients referred to the Transfer of Care Hub thus preventing discharge delays for Wigan residents.
- Co-ordinate discharge of patients referred to the Transfer of Care Hub to facilitate timely discharges for patients. By ensuring that efficient and effective processes are in place to prevent delays in discharge.
