Job summary

To work closely with the multidisciplinary team including specialist palliative care providers to ensure high quality, holistic care delivery throughout palliative stages of illness through to end of life. The post holder will manage a caseload of patients with palliative needs and require symptom management and psychological support/advice to meet their individual needs. The post holder will support patients on discharge from hospital including rapid discharges and will aim to prevent unnecessary admissions to hospital through holistic assessment within community if deterioration occurs.

We have 2 x part time posts (32.5 hours). These are permanent posts to be worked over 7 days but Saturday/Sunday shifts are on alternate weekends and end at 1400.

NHS/UK healthcare experience is essential as is the ability travel across Trust independently. You must be able make multiple domiciliary visits every day whilst transporting all required clinical equipment.

Main duties of the job

  • To attend arranged daily multi-disciplinary team meetings
