Job summary

An exciting opportunity has arisen to join our dynamic district nursing team within the Out of Hours District Nursing service.

We are seeking a highly motivated and forward thinking registered nurse with community experience to assist the Out of Hours District Nursing Team in the delivery of urgent high quality patient focused care to patients in their own homes.

The role requires excellent communication and problem solving skills. Experience of palliative and end of life care is essential to the role. The ability to work as part of a team and to have an adaptable approach to work is essential. The role will also involve providing supervision and support to junior staff, as well as incident processing and effective caseload management.

The service operates a 7-day service, and this role is part-time (33 hours) over 3 night shifts. The role requires an Enhanced DBS with Adults Barred List check, which will be obtained during pre-employment checks.

Will be required to work across the borough delivering care to patients in their own homes. Will be required to deliver care to COVID positive/suspected positive patients as well as high risk patients.

Full driving licence with business use on car insurance essential.

Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh.

Candidates will be asked to provide original qualification certificates to the interview panel

Main duties of the job

To manage a service caseload of patients registered within a local borough. Delegate the workload to the Community Nurses and HCAs. Will work under the direction of the District Nurse Team Coordinator, and be responsible for the assessment of patient's needs, to develop, deliver and evaluate programmes of care. Supports the care management of complex cases. Supports leadership and development within the team. Mentoring pre and post registration students. Liaises with all healthcare professionals and other agencies within the wider community to ensure effective service delivery and an integrated care approach.
