Job summary
A rare and exciting opportunity has arisen within our Home First Team based at Royal Albert Edward Infirmary (RAEI). We are looking to recruit enthusiastic, experienced and highly motivated Therapy Practitioners to work with our already established team of Occupational Therapists.
You will work alongside our Occupational Therapists to deliver Home First assessments at the point of discharge from hospital. This will provide you with the opportunity to follow your patient through their journey from hospital to a safe discharge home.
You will be supported by our qualified Occupational Therapists who will nurture your development and learning with lots of opportunity to enhance your existing skills, knowledge and experience.
This is a full time role (37.5 hours) Monday to Friday but may extend to weekends in the future.
Main duties of the job
Within this role you will support the Occupational Therapists to plan and conduct assessments for a wide range of patients with differing physical, functional and cognitive/mental health needs within their own homes. You will have opportunity to work independently, under direction and supervision to follow up patients post discharge. The role may involve checking on patients progress, adapting any prescribed equipment and provide therapist led interventions. You will have excellent communication skills to enable you to provide feedback to the therapist and excellent documentation skills to evidence your input with patients.
You must be enthusiastic, flexible and organised with a can do attitude to the role and have a clear understanding of Home First principles to discharge.
You will work well individually and as part of a team. You will be required to work across the Wigan borough visiting patients in the bedded areas and their own homes so you must able to travel across Trust independently.
An Enhanced DBS with Adults' Barred List checks will be obtained during pre-employment checks.
This post involves contact with patients and families face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.
