Job summary

An exciting opportunity for a Band 6 Sleep Service Nurse / Practitioner has arisen within the Sleep Medicine Department at Wrightington, Wigan and Leigh Teaching Hospitals NHS Foundation Trust.

Hours are Monday-Friday 8am-4pm within the Sleep Medicine Department outpatient clinics.

We are looking for a highly motivated and dynamic practitioner with relevant experience within the speciality to join the team. This is a rapidly developing and expanding service that must operate in a highly efficient manner to enable the sleep pathway to achieve Government access and treatment time targets, whilst providing the best possible care for the patients.

The sleep service is a busy department and part of the respiratory multidisciplinary team. We have recently gone live with our increased Community Diagnostic Centre activity which the post holder will be part of.

We have a large cohort of patients from assessment, investigation and diagnosis through to provision of treatment, clinical and technical follow up, providing long term supervision where appropriate. The team are dedicated to delivering the best possible care for our patients and aim to continuously improve and develop the service in line with new research ensuring safe and effective practice.

Main duties of the job

The post holder will be responsible for the planning and delivery of care for patients with sleep related breathing disorders and provide long term supervision where appropriate in nurse/practitioner led clinics.

They will promote and deliver high standards of patient care; make decisions based upon knowledge and experience in respect of patient care and be accountable for those decisions and assist in the smooth running of the service and developing specialist programmes of care for patients, which include treatment plans, teaching packages discharge plans and supervising junior staff in the delivery of care.

They should deputise for the senior team as requested at meetings and other events.

  • Assisting in the management of the sleep service, co-ordinating the activities of the service and deploying resources effectively.
  • Support the ongoing development of the Non-Invasive Ventilation service
  • Together with and under the supervision of the senior team, develop specialist programmes of care for patients, which include treatment plans, teaching packages discharge plans and supervising staff in the delivery of care.
  • Actively supporting patients and their families, through communicating difficult and complex information using a variety of methods, to ensure effective understanding.
  • Proactively supporting patients and carers in coming to terms with their illness/condition, providing ongoing support
  • Ensure that effective communication is maintained both within the Team itself and with relevant external agencies or individuals
  • Organise own workload and adjust priorities in response to issues arising in the workplace
  • Recognise and respond appropriately to issues which require urgent or immediate attention, reporting to the other members of the multi-disciplinary/management team as necessary.

  • Bring any potential efficiency savings or anticipated budgetary pressures to the attention of the AGM responsible for the Sleep Service

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.
