On the virtual ward we are committed to provide a high-quality service for patients enabling hospital level care in the home using current health remote monitoring service.
Patients can be admitted to virtual ward to shorten length of stay in hospital enabling early transfer home before they are medically fit for discharge. 923 Patients admitted since January 2022
Alternatively, patients can be admitted directly from home as an alternative pathway to hospital admission.
Capacity at present 70 virtual ward beds increasing to 150 after recruitment drive.
Hours of work 8am-8pm Monday to Sunday

Main duties of the job
An exciting opportunity has arisen with the investment of Virtual Care Technology at WWL, to join an innovative Virtual Hub team, that enhances hospital-without-walls care provision close to home. Virtual Care in the Community allows our patients to receive the care they need in their own home, safely & conveniently as an alternative to hospital. Using community wrap-around services our aim is to provide opportunities to prevent hospital admission, facilitate early discharge, ability to detect deterioration earlier in health condition to prevent readmission to hospital.
We are looking for forward thinking, accountable individuals that excel in working as a team whilst maintaining autonomy, striving to deliver compassionate patient-centred remote monitoring and care management for the Wigan borough population.
This post is permanent full time role (37.5 hours) across Monday to Sunday with no on-call requirement. Part-time will be considered. Applicants must have active/pending NMC registration as well as acute experience, ideally within the NHS.
