The Children and Young Peoples Autism Service have an exciting opportunity for a Neurodevelopmental Pathway Navigator to join the service.
The ND Pathway Navigator will offer needs-led support and signposting to families and young people within the neurodevelopmental pathways/service, this includes the well-established CYP autism service and the newly developing ND pathway including ADHD.
In the role you will ensure that the children and young people are well while waiting for their Neurodevelopmental assessments by signposting parents, carers and young people to further support.
You will also liaise closely with the internal and wider multidisciplinary teams to ensure the family and young person have the correct support in place.
This post is based in Anglesey House, Rugeley, but will include some potential for home visits to locations across South Staffordshire.
If you have experience of supporting family/carers/children and young people coping with autism or previous experience of working with children and young people with Autism and/or ADHD we would love to hear from you!
Main duties of the job
- To provide guidance to Family carers in order for them to access timely support and understanding of the assessment pathway process.
- Support the team to ensure that families are kept informed of the support and options available to them and supporting them to make informed choices on how they and their child or young person are supported.
- Support the understanding of the autism and other ND assessment processes with external agencies, partner organisations and referrers.
- Support children, young people and family/carers to understand the autism and other ND assessment pathways through the provision of information and advice and enhance connection to the local communities.
- Main point of contact for families with a child or young person on the Autism and other ND pathways and proactively aim to offer contact as required, individualised to the needs of the child, young person and family/carers.
- To meet regularly with supervising clinician to review clinical issues, and with team leader for managerial support.
- To work independently with children, young people and families/carers in a variety of settings
