An opportunity has arisen to recruit a qualified Band 8a Advanced Clinical Practitioner (ACP) within Oak ward at the Redwoods Centre in Shrewsbury. This is a fixed term 12-month position to cover maternity leave.

We are seeking a dynamic, highly motivated ACP who ideally has experience of working within an Acute ward environment and ideally experience in working with older adults/frailty and dementia.

You will be part of a multidisciplinary team providing care to service users on Oak Ward (Mixed sex older adult/dementia ward). You will work collaboratively with the multidisciplinary team and senior clinical leads and other ACP's in the development of advanced clinical practice across the service and in the delivery of high standards of safe and quality care.

You will work as an autonomous practitioner, utilising advanced knowledge, skills and expertise in relation to the assessment, diagnosis, treatment and management of a range of physical health and mental health conditions.

This exciting opportunity supports working within the four pillars of advanced practice: - education, clinical practice, leadership and research.

Main duties of the job

  • Receive patients with undifferentiated and undiagnosed problems and make an assessment of their health care needs, based on highly-developed nursing knowledge and skills, including physical examination skills.
  • Plan and coordinate the physical healthcare needs of inpatients on the ward; including the requirement for physical examinations and investigations.
  • Order necessary investigations, and provide treatment and care both individually, as part of a team, and through liaison with / referral to other agencies, e.g. ECGs, Clozaril Bloods.
  • Plan, develop and manage specialist packages of care, employing the Care Programme Approach (CPA), working in partnership with others, delegating and referring as appropriate to optimise health outcomes and resource use.
  • Draw on a diverse range of knowledge in making decisions to determine evidence-based therapeutic interventions (which will usually include prescribing medication and actively monitoring the effectiveness of therapeutic psychological interventions).
  • Use professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery.
  • Undertake weekly assessments of all inpatients under their care, addressing issues and assessing and managing risks identified.
  • Prepare reports encompassing medical and advanced nurse practitioner interventions in readiness for CPA meetings.
