Want to work with D2A pathway 2 beds in an Intermediate Care setting, contribute to a P1 rota to reduce hospital admissions, and see patients for rehab at home following hospital discharge?

Main duties of the job

We are looking to fill a part time band 6 role working across Rapid Response, Community Rehab and D2A Pathway 2 Rehab beds as part of the Community Therapies Service in East Staffordshire. You will also have the opportunity to contribute to the development of an expanding and developing service, great for building leadership skills and providing support and supervision to band 5 OT's, assistant practitioners, rehab assistants and students as required.

This post is agile working in the local community, in patient homes and Care Homes and the ability to travel independently across East Staffs is essential.
