Job summary
The 0-19 Stafford & Surrounds team are recruiting aCommunity Development Practitioner / Specialist Public Health Nurse based in Stafford working 22.5 hours per week
You will be a qualified Health Visitor or School Nurse and will work autonomously and collaborate with partners in other services involved in locality community initiatives and community development in order to promote and facilitate the shift of provision of health services offered from the 0-19 service to be community focused where it is appropriate to do so and where they are not currently.
You will act to support best practice of service provision with partners and act as a resource regarding community initiatives for the locality 0-19 team. You will plan, deliver and evaluate training and update study days to service users and partners where this is required to deliver community focused initiatives.
Main duties of the job
- To adhere to and apply the code of ethics and professional conduct and all other relevant guidance from the Nursing & Midwifery Council (NMC) and the Partnership Trust. To be professionally accountable and responsible for all aspects of own work.
- To establish and maintain effective working relationships both within the Partnership Trust and with external organisations in order to meet service requirements and support and demonstrate the Trust's equality values.
- To assess service user referrals and care/support needs. Assesses service users and formulates, develops, implements and evaluates programmes of treatment /care/support for individuals, families and carers. Takes a lead in community development and provides advice and support to others. The specialist care/support plans /developments may include the following:
o Promoting independence/self-management and maintaining safety o Increased functional ability
o Improvement in health and wellbeing
o Interventions in disease/condition management
o Prevention and reducing of health inequalities
o Admission avoidance
o Case management/ treatment /care plans
o Palliative and end of life care
o The safeguarding and protection of children, young people and families.
o Raise awareness and empower staff to support parents in their vital role in their child's development and their ability to influence healthy lifestyle choices.
