Mental Health Nurse – Older Adult Community Services South East Staffs

Job summary

We are offering an exciting opportunity to join our Dementia Team and the Older Adults Mental health Team 6 Community Mental Health Nurse based at St Michaels Court in Lichfield and working across South Staffordshire.

You will play a pivotal role delivering high-quality clinical services to service users and their families. You will work closely with other providers in line with the recommendations from National Strategy and appropriate NICE Guidelines. You will be part of a service that provides evidence-based, first-class healthcare putting service users' needs at the centre of the service.

Your role also extends to the Older Adults Team, working with Staffordshire Police and Mental Health Services, embodying the Trust's commitment to people-centric care and helping shape the well-being of the elderly population in South Staffordshire. Leverage your extensive experience in older adult and dementia care, conducting assessments, formulating person-centred care plans, and managing risk assessments.

You will need to work flexibly to meet service user, carer and family needs. The work patterns and client base is unpredictable and there is regular exposure to highly distressing and emotional circumstances.

Main duties of the job

As a band 6 Community Mental Health Nurse you will be part of a service that will provide specialist input to service users and their carers who may be affected by a mental health diagnosis.As part of this role, you will be expected to:

  • Have a level of knowledge and expertise acquired through in-depth experience of older adult mental health and dementia care underpinned by extensive post registration to develop skills, knowledge and an excellent values base.
  • Work autonomously on completing the assessments and formulating person centred care plans and risk assessments for those accessing the specialist service.
  • Be responsible for the delivery of a range of high level specialist assessments, identifying risks and management plans in the process.
  • Provide indirect care through the functions of educator and advice giver to relatives, carers and other organisations where appropriate and play a significant role in the supervision of junior staff.
  • Support and advise junior staff.
