Job summary
We are offering an exciting opportunity, ideal for a newly qualified Occupational Therapist to join our Home First, Discharge to Assess (D2A) Service and Haywood Hospitals Teams on a rotational post basis.
This is an excellent opportunity to gain the appropriate skills and expertise to widen your career opportunities through working in a range of specialties and settings within the services in D2A and the Haywood Hospital.
As part of your rotational post you will have a unique opportunity to work within inpatient & community care settings, learn and develop advanced skills and knowledge across a wide range of services.
You are required to hold a full HCPC Registration and as a newly qualified Occupational Therapy practitioner we offer an extensive Preceptorship Program which you will engage on throughout your rotation.
Due to the nature of this post you will be required to adequately travel across the geographical area covered by our services in North Staffordshire to meet the demands of the service.
To assist in the development of a flexible service shift patterns may include evenings and weekends, shift pattern over 7 days, early (9-5pm and lates 12- 8pm) Sunday's are 10am - 6pm.
Main duties of the job
Working across all rotations you will deliver a range of interventions following specific Occupational Therapy assessments to enable understanding of any expression of an unmet need and to maximise an individual's abilities.
As part of this role you will be expected to:
- Take responsibility for the management of a caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.
- Carry out assessments including gathering complex information from Service Users, carers and other members of the multi-disciplinary team. This may include the use of standardised assessment tools, verbal and non-verbal communication skills and observation of the Service User and their environment.
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.
- Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
- Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
