0-19 Practitioner- Health visitor/School Nurse -Strengthening Families

We are looking for a highly motivated Health Visitor or School Nurse with excellent team working and communication skills to join the Families' Health and Wellbeing 0-19 Service in the role of Strengthening Families Practitioner in ourCannock and Rugeley 0-19 team. This vacancy is for 22.5 hours each week and is based at Norton Canes Health Centre.

The Families' Health and Wellbeing Service (0-19) in Staffordshire offers an integrated, flexible and family focused service providing high quality, compassionate, personalised support and care to Children, Young People and Families across Staffordshire.

As a Strengthening Families Practitioner, you will have focus in the partnership plus agenda, working closely with partner agencies across health, social care and the local authority to support families with higher levels of need and to enable each child to reach their developmental and educational potential.

Main duties of the job

  • Be a Specialist Community Public Health Practitioner and have detailed professional and clinical knowledge.
  • Have a focus in the partnership plus agenda, as well as contributing to the public health across the 0-19 agenda.
  • Be responsible for a defined population of service users in a variety of settings across the Partnership Trust including the community, family hubs and schools/educational establishments.
  • Be skilled in the assessment of children and young people's care/support needs and in the development, implementation and evaluation of care/support
  • Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/improvements.
