Band 7 Reconnect Team Lead

Job summary

This is an exciting opportunity to lead the Reconnect team within West Mercia Health and Justice service.This role will give you the opportunity to have a varied week spending time in the areas that the team cover, demonstrating a visible presence leading the team. There will be a requirement for you to have equal time based within HMP Hewell, HMP Stoke Heath and the Community bases - main base for the community is Church Street, Kidderminster. There will be no remote working as part of this role.The Reconnect Pathway consists of providing assessments to people in prison up to 12 weeks prior to their release, referrals on for those out of area and community support for those people released into the West Mercia area.The Reconnect service will be delivered to people who present with any health vulnerability including but not limited to; mental health, physical health, learning disability, head injury etc.The Team Lead will be invaluable in promoting the service with stakeholders, showcasing the excellent work to commissioners and leading ongoing quality improvement within the service.The successful applicants must be experienced in risk assessment, supervision, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.Due to the nature of the work posts will be subject to Prison vetting.

Main duties of the job

**Please refer to the attached Job Description in summary:**

o The post holder will provide leadership, professional supervision & expertise when assessing vulnerabilities / complex needs within the Criminal Justice System; including expertise of the Mental Health Act (Civil / Forensic Sections).

o There is an expectation that all work is underpinned by theoretical and practice knowledge, and delivered to a high standard in line with statutory procedures and professional frameworks.

o The post holder will be responsible for providing advice / support to Practitioners.

o Where required, post holder will be required to complete specialist assessment and interventions to people with a diverse range of needs coming into contact with the Criminal Justice System via the Reconnect team.

o The post holder will also be responsible for supporting / advising colleagues regarding complex needs.

o The post holder will also be expected to be available for consultation, support and clinical supervision to other health and social care professionals working within Liaison and Diversion Team.
