We are offering an opportunity for Band 5 NMC Registered Staff Nurses to join our Community nursing teams in North Staffordshire. Opportunities currently available in our services includes:
- ABC PCN based in Bradwell Hospital
- Newcastle Central PCN based in MilehousePrimary Care Centre
- Whitfield PCN based in Tunstall Primary Care Centre
We are looking for nurses who are committed to providing high standards of holistic care to join our busy, supportive & friendly teams. This is an excellent opportunity for highly skilled nurse looking for a new challenge or looking to return to Community Nursing.
Many people would be unable to live at home without the support they receive from the Community Nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice.

This role includes daytime working, including weekends and cover of early morning shifts starting from 7am when required.
Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.
Main duties of the job
As a highly skilled NMC Registered nurse you will work in collaboration with the muti-disciplinary team and colleagues to:
Promote independence and support patient centred care planning.
Provide care directly to patients on the caseload with a range of nursing needs (palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management)
Work autonomously, in line with your scope of practice, competence and policies.
Handover patient information to the wider team, document patient information effectively.
Escalation of patients' needs as appropriate.
Lone working with the support from the wider team.
