We are delighted to offer an exciting opportunity to join our Countywide Community Respiratory Service as a NMC Registered Community Respiratory Nurse.

We are looking for a nurse who has a clear passion, drive and ability to deliver high quality person-centred compassionate care. You will utilise your advanced skills to work in a variety of settings, including clinics and the patient's home providing a comprehensive and holistic service for patients with a confirmed diagnosis of respiratory disease.

As a community Respiratory Nurse you will work closely with the multi-disciplinary teams / agencies in addressing a range of complex care / issues across the spectrum of Respiratory Disease management; preventing unnecessary hospital admissions and improving the health and wellbeing of the patients in our care.

Due to the nature of this role you will require suitable and adequate transport to work across the geographical area to provide essential community care.

Main duties of the job

With strong communication and organisational skills and a flexible approach, you will work collaboratively with patients and key partners to facilitate the best possible patient experience and outcomes.

As part of this role you will be expected to:

  • Demonstrate the ability to effectively and efficiently assess, plan, provide and evaluate evidence based nursing care for patients.
  • Demonstrate strong clinical reasoning and prioritisation skills and be able to work effectively, both independently and as a member of a team.
  • Have a commitment to professional development and contribute to a culture of continuous improvement.
  • Organise and prioritise your own day- to-day work tasks and activities and delegate tasks to junior colleagues.

  • Post Graduate studies in relevant nursing field such as; end of life care, long term conditions/palliative care or related/relevant subject
  • Mentorship qualification (998-997)
  • Post qualification courses in nursing
  • Attendance at relevant networks e.g. Special Interest Groups
  • Up to date field work/pre-registration student training

Desirable

  • Modules at Masters level
  • Evidence of further clinical study

Experience

  • Relevant experience to the role including assessment and management of respiratory and long term conditions
  • Ability to manage time and caseload effectively
  • Supporting service users, families and carers in dealing with admission to hospitals and ill-health/working in a multidisciplinary team

  • Experience of negotiation and conflict management in clinical situations
