Job summary

The Community Children's Nursing Specialist and Family Services team has an opportunity for a Band 6Community Children's Nurse (CCN) to their team in Fenton.

The Community Children's Nurse (CCN) will manage a defined caseload of CYP with a range of acute and complex health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.

The service aims to support families in caring for their sick children at home, in order to avoid hospital admission and/or facilitate early discharge. This is done by the assessment of children and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.

In the role you will have responsibility for the management of a defined caseload and responsibility for the team caseload including out of hours/on call when required. The team provides a 7-day week service to include 24 hours on call for children receiving palliative care.

You will work in partnership with health colleagues and other agencies to provide potentially complex packages of care.

Main duties of the job

  • Plan and organise own caseload in order to meet the needs of the children and families.
  • Develop, assess, implement and evaluate plans of care working in partnership with children and families
  • Provide specialist knowledge and skilled nursing care to children and families in their own home or community setting
  • Be able to work flexiblyand participate in on call rota to provide 24 hour cover if needed.
  • Establish and maintain links with professionals and agencies involved in the child's care, possibly taking on responsibility of key worker when appropriate.
  • Work in partnership with all concerned to ensure smooth transition between hospital and home to include discharge planning.
  • Identify and report any cases of suspected child abuse in accordance with Staffordshire Safeguarding Children Board (SSCB) guidelines and 2004 Children Act, and attend core group and case conferences, preparing statements and reports as appropriate.
  • Assist in planning and organisation of individual therapeutic programmes of care for children and young people by attending assessment, discharge planning and safeguarding meetings, ensuring reports are submitted on the child's progress if unable to attend.
