We are offering an exciting opportunity for a senior nurse or allied health professional to join The Rehabilitation Medicine department based at the Haywood Hospital in Burslem, Stoke on Trent as a Complex Discharge Lead for a 12 month period.

Taking on the role of 'Navigator' leading the coordination of the patients efficient journey through their Specialist Rehabilitation inpatient stay on Broadfield Unit from admission to discharge, providing assurance for timely flow within the whole pathway, ensuring compliance with NHS England (NHSE) service specification around discharge planning.

You will need to demonstrate advanced skills and expert knowledge to provide comprehensive assessment of the patients biopsychosocial needs, developing and implementing standards and protocols to support up to date evidence based practice in line with clinical guidance and national standards relevant to the patient cohort.

You will act as a highly specialised source of expertise on the management of service users within the specialist area, developing and fostering productive working relationships with local and national stakeholders to achieve the best outcomes for services users and improvement and development of pathways.

Main duties of the job

You will be responsible for :

Navigating and determining the most appropriate care setting and funding route for discharge based on the best available evidence, utilising the skills of the multidisciplinary team.

Dealing with highly contentious issues regarding decision making for ongoing management of rehabilitation and care; dealing with highly sensitive issues around expectations of the service, especially where there are significant barriers to understanding.

Supporting service users who lack mental capacity with complex decision making for discharge planning, ensuring that service users are at the heart of decision making.

Using negotiation and interpersonal skills to support and manage/defuse potentially hostile and antagonistic situations with staff, service users and family/carers in potential highly emotive atmospheres where there are conflicting views.

Playing a supervisory role to the discharge facilitator, supporting competencies as appropriate to the requirements of the role and knowledge of the population of the service user to facilitate assessment and discharge planning and prevention of delays.

Support, educate and facilitate continual learning for staff in the processes of complex discharge, goal planning, mental capacity issues and specialist rehabilitation, ensuring best practice that reflects the values of the service and the Trust.
