The Triage Nurse will provide triage assessment, clinical support, assistance and professional expertise to all health care professional referrers requiring assistance and support with Admission Avoidance and hospital discharges.

The Service will be the access point for all community nursing referrals for the Locality referrals professionals requiring Health or Social Care Services.

The triage nurse will provide clinical triage and support where appropriate. This may range from signposting the referral caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment.

The post holder is expected to represent the organisation in a professional and courteous manner at all times and provide advice and information using a level of clinical understanding and professional judgement.

To establish and maintain excellent communication links with all Health and Social Care Professionals.

To work independently, exercising assessment and evaluation skills to take a clear and comprehensive referral, for the community nursing teams in the locality obtaining all relevant documentation as appropriate for referral.

Main duties of the job

To control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner

Work as a member of the nursing team in conjunction with other professionals to promote seamless patient care.

Be prepared to work flexibly, in the interests of self-development and exigencies of the service. To actively participate in the on-going development of the role and support the development of the protocols and processes within the service.

To possess the ability to prioritise tasks, cases and situations appropriately to ensure effective delivery during periods of high demand.

To liaise and communicate clinical information verbally and/or via technical links to healthcare partners to ensure continuity of care for the caller

To refer any issue or concerns outside the scope of post holders responsibility or professional registration promptly to the Operational Lead/Manager.
