Job summary
Community Matrons provide case management to a group of patients with complex long term health conditions that require skilled intervention. Understanding long term conditions is a major contributing factor to this role, supporting self management and self-recognition of decompensation and acting in a timely manner. Working as part of an MDT is essential.
Main duties of the job
Care and support of patients with complex long term conditions
Managing and supporting patients to self manage these conditions being aware of decompensation of disease and illness.
Use physical assessment skills to support long term condition management and be able to use diagnostic skills to then provide a treatment plan if required using V300 prescribing skills.
work with other members of the MDT including the Community Nursing Teams and GP.
