Job summary

We are a well-established, supportive and friendly Community Nursing team excited to be offering a great opportunity in Ambulatory and Wound Care Clinics. We have a vacancy available within our Stafford locality clinics.

You will have a commitment to providing holistic care to patients in a clinic setting, Be able to work on your own initiative and work well within a team. Key clinical skills include complex tissue viability issues, wound care, bladder and bowel management, compression therapy as well as medication administration.

Within the community we are seeing more complex and acute patients than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Working within our Community Nursing Ambulatory Team you will support, assess, and treat patients with a variety of health needs, delivering high quality, evidence based practice.

Due to the nature of community working, the ability to travel across the patch to alternative clinics or patients own home is essential.

Main duties of the job

Working in collaboration with the multi-disciplinary team and colleagues to:

  • Promote independence and support patient centred care planning.
  • Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).
  • Work autonomously, in line with your scope of practice, competence and policies.
  • Handover patient information to the wider team, document patient information effectively.
  • Escalation of patients needs as appropriate.
