Job summary

We are seeking to recruit an energetic, flexible and enthusiastic Mental Health Nurse to join the Crisis Resolution and Home Treatment Team (CRHTT) based at The Redwoods in Shrewsbury.

You will be part of a dynamic multidisciplinary team providing a 24-hour rapid response service, delivering assessment and intervention for individuals experiencing an acute mental health crisis in the community. This includes offering short-term intensive support and acute psychiatric care within service users' homes or other community settings, with the aim of resolving the crisis and avoiding unnecessary hospital admission.

You will contribute to the delivery of the agreed aims and objectives of the service and identify any factors that may inhibit their achievement. You will assist in delivering a high standard of nursing care to service users referred to the team and will implement agreed systems for monitoring the effectiveness of care and treatment.

Band 5 nurses play a crucial role in delivering safe, effective, and recovery-focused home treatment for an identified client group, in line with each individual's wellbeing and recovery action plan.

The post requires you to travel independently across a large geographical area, therefore suitable transport is essential.

Main duties of the job

  • Working in an identified adult community mental health pathway, with the flexibility ofworking across pathways, dependent on skill and service need.
  • Delivering a high-quality clinical service to patients and their families.
  • Care coordination of an identified caseload with access to regular and robust
  • Delivering occupational therapy interventions, in accordance with an agreed careplan, to an identified group of people experiencing mental health related difficulties.
  • Communication skills are an integral part of the role. Written and verbal skills will be vital in ensuring that information is documented, shared and recorded in an accurate and prompt manner.
  • It is essential that you possess the skills to work as part of a team and have the confidence to work autonomously. You should possess the skills and enthusiasm to take on the challenge of a changing, modern day community mental health service.
  • Having proficient IT skills and being a car driver are essential.
