Band 4 Care Act Assessor - 12-month contract for Stafford Adult Social care
An exciting opportunity has arisen for a Care Act Assessor post on the Stafford Initial Response team - 30 hours per week
We are looking to recruit a Care Act assessor to cover maternity leave and to work within the Adult Social care team in Stafford. The position is within the older persons with physical disability team which is a community-based team working with adults over 18 years of age with a range of physical and Mental health needs.
Main duties of the job
As a Care Act Assessor on the Initial response team, you will complete holistic assessments and reviews for individuals within the Stafford area, using The Care Act 2014 to consider eligibility and wellbeing. You will identify any needs and gather information to ensure that needs are being met in the most appropriate way and that outcomes are being met, including considering appropriate services to meet the needs of individuals. You will be responsible for your own caseload; visits will be within peoples own homes as well as care settings such as residential, nursing, and extra care.
The post will involve travel and so a driving licence and access to a vehicle are crucial
The post holder will be expected to access Social care and health records to gather information, therefore it is important for the post holder to be able to use a computer and understand the importance of good recording. You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals. You will be required to be self-motivated and organised to manage any competing demands and be provided with all relevant training and support in the role.
