Pathway Lead - Older Adults Community Mental Health Team

Job summary

An opportunity has arisen to recruit x 1 band 7 Mental Health Pathway Lead within the Older Adult Community Mental Health team, clinically working in the South Staffordshire West locality.

Applicants should possess the knowledge and skills to enable them to work in older adult mental health speciality, working with service users affected by all types of functional mental health and Dementia conditions. This includes working with challenging and complex needs.

Applicants will be required to work closely with the existing multi-disciplinary team members and working within a clinical and leadership role. The role will include close working with physical and social care colleagues along with our GP and primary care colleagues.

Main duties of the job

  • Delivery of a high standard of nursing care to individuals referred to the Older Adult CMHT
  • To provide daily operational/ clinical leadership and supervision to the neighbourhood teams.
  • To participate actively in Collective Leadership and Quality Improvement projects relating to the services.
  • Provide a specialist mental health nursing assessment, care planning and to evaluate those experiencing functional mental health conditions.
  • To network and work in partnership with fellow team members and internal/external partners.
  • To undertake their own professional development and compliance with mandatory training.
