Midlands Partnership University NHS Foundation Trust has an opportunity to join our Crisis Resolution/ Home Crisis Treatment team in Southwest Staffordshire based at Haywood Lodge, Stafford as a Community Mental Health Practitioner.
We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week.
This is a 24-hour service and team members are all are required to work rostered shifts including weekends and nights and you must have access to suitable transport, to cover a large geographical area for your shifts.
Main duties of the job
- Assessing mental health crises in individuals referred to the service and supporting the delivery of safe and effective home treatment for an identified client group and in accordance with a wellbeing and recovery action plan
- You will participate in the delivery of the agreed aims and objectives of the service and identify factors which may inhibit these from being achieved.
- You will assist in the delivery of a high standard of nursing care to service users referred to the team.
- You will implement agreed systems for the monitoring of effectiveness of care.
- You will participate in the education and management of junior staff.
