North Shropshire Community Mental Health service provides assessment and evidence-based interventions for people who presenting with complex mental health difficulties that are significantly impacting on their daily life alongside our partner organisations.

We are looking for a Senior Nurse Practitioner to the join our team to work alongside our secondary care mental health practitioners and wider partners to support the delivery of our service.

You will need to encourage effective engagement of those presenting with complex mental health needs, utilising excellent communication skills, whilst maintaining compassion and hope through every contact. We incorporate a trauma informed psychological approach which aims to increase people's independence, working towards an improved quality of life.

The post will be split between our sites in Oswestry and Market Drayton

Main duties of the job

  • Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administer medication,complyingwith relevant Trustprocedures and NMC Guidance.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

You will be asked within this role to support and contribute to the clinical supervision of junior members of the team and students on placement with the service
