An exciting opportunity has arisen for a NMC Registered Band 5 Nurseto join theHome First Team (Discharge to Assess Service)in Longton, Stoke on Trent.
The team supports patients in the community to prevent hospital admissions or assist with discharge following acute hospital care. We deliver high-quality care to individuals with complex needs, promoting independence, rehabilitation, and self-care.
Our ethos is to enable safe discharges and provide nursing support at home, helping patients remain in their own environment and avoid unnecessary readmissions.
As a valued team member, you'll work collaboratively within a multi-disciplinary team and with Operational Leadership to ensure excellent care delivery. You'll carry out specialist assessments and care planning with service users and carers.

This role involves working shifts across a 24/7, 365-day schedule and requires independent travel across a large geographical area. You may be required to work from any of the three North Staffordshire bases.
Main duties of the job
As a Band 5 Nurse you will provide holistic assessment, planning, implementation and evaluation of care as part of the D2A process.
As part of this role, you will be expected to:
- Act as a role model and motivate team members in working towards innovative and high standard of practice.
- Participate and support the Senior Nurse Coordinator and Clinical Lead in the development of an effective team and the development of productive working relationships throughout the Trust.
- Support the Senior Nurse Coordinator and Clinical Lead in the development of a positive and 'supportive' team culture.
- Providing public health, health promotional programs to improve health and reduce inequalities.
- Be responsible, and accountable, for service delivery to clients/patients.

- Improvementinhealthandwellbeingincludingmentalhealth.
- Earlysupporteddischarge.
- Be able to initiate referrals to other health professional specialist services and agencies. To establish and monitor good working relationships with other health and social care professionals, statutory and voluntary organisations.
- Provide patients and relatives with information and education thus ensuring they have meaningful choices that promote dignity, independence and quality of life
- Advise Senior Nurse Coordinator and Clinical Lead of any issues that may affect the patient or the service.
