Job summary

We are seeking a dedicated and compassionate nurse to join our Community Paediatrics Team, supporting the delivery of a high-quality service for children, young people, and their families. This role focuses on providing health assessments, investigations, and treatment for children and young people with long-term health conditions and neuro-disabilities.

As part of this role, you will lead nurse-led clinics for children with identified long-term conditions, offering expert care, advice, and education to help families manage their child's health effectively. You will also play a key role in supporting and mentoring junior staff and learners, ensuring the consistent delivery of safe, evidence-based, and person-centred care.

Collaboration is at the heart of this position--you will work closely with multi-disciplinary team members and partner agencies to ensure that care is well-planned, coordinated, and responsive to the needs of each child and family.

Main duties of the job

Deliver high-quality, evidence-based nursing care and health assessments for children and young people with long-term conditions and neuro-disabilities.

Manage an independent caseload, providing specialist advice, onward referrals, and clinical information to support Education, Health and Care Plans (EHCPs).

Work collaboratively with multi-disciplinary teams and external agencies to ensure effective care planning, safeguarding, and transition to adult services.

Provide leadership, mentorship, and supervision to junior staff and learners, promoting professional development and maintaining clinical standards.

Maintain accurate patient records, ensure safe use of resources and equipment, and contribute to clinical governance, audit, and service improvement.

Demonstrate flexibility and professionalism, promoting equality, diversity, and safety while adapting to changing service needs across multiple sites.
