Senior Community Mental Health Nurse

We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Nurse to join our Older Adult Community Mental Health Team within the Shropshire, Telford and Wrekin locality.

This is an exciting opportunity to be part of a dynamic and supportive multidisciplinary team delivering high-quality care to older adults experiencing functional and/or organic mental health needs. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.

As a Mental Health Practitioner within the Older Adult Community Mental Health Team, you will be responsible for delivering comprehensive assessments, risk formulations, and evidence-based interventions to support older adults with complex mental health needs.

The role involves working closely with colleagues across the multidisciplinary team and liaising with GPs, social care, and voluntary sector partners to ensure continuity and quality of care. You will maintain accurate and timely clinical records in line with Trust policies, in addition you will contribute to service development, quality improvement initiatives, and provide professional advice, support, and supervision to colleagues where appropriate.

Main duties of the job

  • Take responsibility, for the management ofa complex caseload, whichincludes, specialist assessment (withinservice specification), care planning in conjunction with service user and carer, evaluation of individualtreatmentplans(CareProgrammeApproachandSingleAssessmentProcess).
  • Responsible for participating in the setting ofquality standards, including the auditing, monitoring and reviewingof practice in line withcurrent clinical guidance practice and policy.
  • Administer medication, complying with relevantTrust procedures and NMCGuidance.
  • Assess patient's individual care needs, develop, implement and evaluate programmes ofcare.
  • Deliverarangeof interventionsinaccordancewiththeagreedplanofcareandPathway guidanceincludingpharmacologicalandpsychologicallyinformedinterventions.
  • Complete and contribute, where appropriate to robust riskassessments and riskmanagement plans. Deliverand promote positiverisk taking, where appropriate, to improvequalityoflifeandmaximiseoccupationalperformance/independence.
  • Maintain an effectivereporting system by observing and reportingverballyand in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.
