Community Mental Health Practitioner (Nurse/OT/SW)

Job summary

Are you a Mental Health Nurse, Occupational Therapist or Social Worker who wants to make a difference to community mental health teams in Telford?

We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Practitioner to join our Older Adult Community Mental Health Team within the Shropshire, Telford and Wrekin locality.

This is an exciting opportunity to be part of a dynamic and supportive multidisciplinary team delivering high-quality care to older adults experiencing functional and/or organic mental health needs. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.

Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;

  • Provide specialist assessment and care planning in conjunction with service users and carers
  • Evaluate treatment plans
  • Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
  • Co-ordinate the work of the Team

Main duties of the job

  • Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process).
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance (if from nursing background)
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

  • A recorded/registered qualification in one of the following in CMHN, Social Work, Occupational Therapy, Psychological Therapy
  • Post qualification experience, such as evidence of formal courses and training relevant to people with complex mental health needs

Desirable

  • Qualification in psychological interventions such as DBT, SCM, CBT, EMDR, MBT, relevant to a client group with complex mental health needs

Experience

Essential

  • Experience of delivering therapeutic interventions to patients with dementia and their carers
  • Experience of assessing, planning, co-ordinating and managing people with mental health problems in a community setting.
  • Experience of working with complex and challenging needs in older adult care

Desirable

  • Minimum of one year's post registration experience working in a mental health setting
  • Experience of supervising junior staff / students
