We are looking for a Healthcare Support Worker to join our established team to deliver Palliative and End of Life Care for patients within the community based in Cannock.

Our service works closely with community services within our Trust, charitable and voluntary services, and care agencies to address a wide range of care/support issues to improve end of life care for patients, their families and carers.

You must be able to demonstrate an understanding of palliative care and possess the dedication and commitment to supporting and caring for individuals, their families and carers. Specific training will be provided to support staff development and ensure a quality service delivery.

Shifts will comprise of 8am-4pm / 10am-6pm, with staff being expected to work flexibly in response to service needs.

Your base will be at Norton Canes Health Centre, Cannock,WS11 9SE and will predominately be supporting patients in the South, East and West of Staffordshire. There will be a requirement to visit the main base at Oak Ward, Bradwell Hospital, Newcastle U Lyme, ST5 7NJ

You will need to be able to adequately travel independently over a large geographical area to meet the demands of the role.

Main duties of the job

As a Healthcare Support Worker, you will be involved in the first point of contact in the care pathway and assist in the delivery of outcome-based goals and self-directed goal planning.

As part of this role, you will be expected to:

  • Undertake person centred support planning
  • Deliver support plans and safeguard vulnerable people
  • Contribute to outcome focused reviews
  • Encourage peoples independence, choice, health & wellbeing
  • Encourage individuals use of equipment and assistive technology
  • Facilitate individuals transition to alternative services and support if needed
  • Engage in organisational learning and development
  • Provide excellent patient care & support
  • Recording with Care - complete documentation and feedback
