We are offering an exciting opportunity for a Band 6 Community Mental Health Practitioner to work with individuals with complex emotional needs based in clinic or community venues within Seisdon.
You would be working within the Integrated Mental Health Team (IMHT) and the The Intensive Life Skills (ILS) Pathway to provide assessment and intervention for adults 17+ experiencing complex emotional difficulties as part of the Adult Mental Health Service. You will work as part of a multi-disciplinary team to provide evidence based interventions.
You will support the service as either a highly skilled NMC Registered Nurse, HCPC Registered Occupational Therapist or Registered Social Worker working within highly psychologically minded, enthusiastic, dedicated and supportive teams to support service-users who present with high levels of emotional distress and often have histories of complex trauma, neurodiversity and/or adverse experiences.
You will need to travel independently over a large geographical area and will require suitable transport.
Main duties of the job
As a Mental Health Community Practitioner you will be expected to undertake holistic comprehensive assessments and formulation of complex clinical presentations, including risk assessments and risk management.
The role will involve:
Delivering group based and individual interventions in line with the pathway model and utilising Structured Clinical Management (SCM). There will be a requirement to undertake the role of named worker for a caseload of service users.
Some individual and group-based interventions are being delivered remotely, and the successful candidate will therefore have strong ICT skills and be able to communicate effectively with others through technology.
Participate in both delivering and receiving a range of clinical supervision and professional and clinical development within the pathway.
Provide consultation, support and clinical supervision to other health professionals working within the Integrated Community Mental Health Services and Older Adults service and other areas of the Trust where identified as appropriate.
