Job summary

The aim of the health and justice service is to focus on reduce the health inequalities for vulnerable people who enter into the criminal justice system.

The Outreach practitioner role is to work in an outreach service within the wider health and justice service. The outreach service will be a new service that is being developed to support vulnerable children and adults who have entered into the criminal justice system. The service will have a focus on supporting adults and children who have been released from police custody and may have additional health needs. The focus of the team is to provide support for a period of up to 12 weeks in the community to reduce risks around reoffending and also support health vulnerabilities.As a Outreach practitioner you will be helping to develop pathways for support for children and young people who have come through the criminal justice system. You will be developing connections with partner agencies and looking at providing support and interventions for children and young people. The role is a new development and it is a good opportunity to be part of a developing service and support the creation of new pathways of support for children and young people.

Main duties of the job

The main duties of the Outreach practitioner will be to work closely within the outreach service and have a focus on adults and children and young people who are being referred on after coming into contact with the criminal justice services. The role is a new role and this will opportunities to think creatively and work in partnership with agencies in developing pathways of support.

To support the outreach service to develop a triage and referral pathway for adults children and young people and have oversight of this development alongside the team leader.

  • Registered Mental Health Nurse / Learning Disability Nurse/General Nurse on NMC Register.
  • Registered Social Worker.
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.

Desirable

  • Leadership qualification.
  • BSc degree in nursing or equivalent level.
  • Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management.
