Job summary

We are excited to offer an opportunity to join our team as a Band 6 Senior Occupational Therapist to work within our existing Integrated Complex Assessment Team (ICAT) based in Cannock covering the Southwest district Discharge to Assess (D2A) beds.

The service provides Rehabilitation, Reablement and Recovery to those individuals who have been discharged from hospital with the emphasis of getting them back to their previous level of independence.

As Senior OT you will have an understanding of detailed theoretical and practical knowledge in one of the following areas of Rehabilitation, Integrated Community, Community Intervention and Enablement. You will be skilled in the assessment of complex care needs and, in the development, implementation, and evaluation of treatment plans and will work with the Team Leader to conduct delegated tasks instrumental in team management and care co-ordination.

You will demonstrate initiative and be creative in finding solutions to problems and will hold supervisory, training & leadership responsibilities where appropriate with some responsibility for team performance & service developments.

Due to the nature of this service, you will be required to travel independently over a large geographical area and will ideally have a valid driving licence and access to a car.

Main duties of the job

As a band 6 Senior OT, you will be working closely with qualified and non-qualified therapists for day-to-day service delivery, with the remit of supporting individuals who have been recently discharged from hospital into a rehab bed in a local care home. Your role will be to maximise individuals' independence in a holistic manner and reduce the need for long term services.

As part of this role, you will be expected to:

  • Be competent in the key elements of the career framework levels 1 to 6 (skills for health).
  • Work with individuals and carers who may find it difficult to engage with the service or aspects of the service resulting in uncooperative or challenging behaviour that has to be managed.
  • To assess residents care/support needs when required, providing advice and support whilst working closely with the care home staff to formulate an appropriate plan of care for residents. Intervention may be required to support with:

o Maintaining safety and highlighting areas of concern

o Moving and handling

o Falls management strategies

o Increased functional and cognitive ability

o Improvement in health and wellbeing including mental health

o Interventions in disease/condition management

o Prevention and reducing of health inequalities

  • To have people management skills which incorporate compassionate and appreciative leadership.
