Job summary
JOB PURPOSE
The post holder will be responsible for working in Health and Justice (H&J) Services, dependent on skill and service need. Health and Justice is not itself a treatment service, but it is an identification, assessment and short-term support service.
There will be Health and Justice Practitioners within Custody Suites, Courts and Prisons (RECONNECT) to identify and assess vulnerable individuals. It uses assessments to make appropriate referrals for treatment and support, and ensures criminal justice practitioners are notified of specific health requirements and vulnerabilities of an individual, which can be taken into account when decisions about charging and sentencing are made.
The post holder will be responsible for providing short term support to vulnerable individuals upon leaving Custody or Court, where further support is required. The post holder will be responsible for working closely with the individual to prioritise need/s and plan actions to address the identified need/s. This may include supporting an individual to make a referral to a specific service and support the individual to attend the initial appointment with that service.
The role will involve ongoing assessment, accessing and implementing a wide and, at times, complex range of care services across an ageless Service.
The post holder will be responsible for carrying out assigned tasks as part of the Health and Justice Service.
Main duties of the job
Working with service users referred to Health & Justice and the individual's carers, helping to promote self-determination, choice and autonomy.
To effectively establish a therapeutic relationship with service users and relatives / carer.
Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
Assess patient's individual care needs, agree and refer on to appropriate agency. Involve the individual in the planning and prioritisation of their activity where possible. Provide short term intervention to encourage individual to engage with services and attending their initial appointments
Utilise agreed outcome measures to review the effectiveness of process, as part of Health and Justice Service.
Participate in the development of the service to ensure clinical governance processes are actively implemented at team level.
Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
To remain up to date and compliant with mandatory training requirements.
Service is based at Crescent House, Bilston - but offers support to service users in a variety of community settings across the Black Country.
