Senior Community Children's Nurse

Job summary

We are looking to recruit enthusiastic children's nurse to join our established Community Nursing team within the Palliative & Complex care element.

The team works in partnership with children and young people (CYP) aged 0-18yrs (possibly up to 25 with SEND), their parents/carers and other professional disciplines to support the care of both short and long-term healthcare needs in the home and community settings.

The whole service is provided across the locality of North Staffordshire, seven days a week 0900-2100 hours, 0900-1700 hours on bank holidays and 24 hour support is offered to CYP and their families during end of life care. Our service has two elements to the service, an acute ( previously known as H@H service ) and a long term palliative and complex caseload ( previously known as the PACCT service), hence working hours for the role being 9-5 Mon-Fri with on call coverage 24/7 when required. This job role is to predominantly cover the long term palliative and complex caseload. The post-holders will be responsible for ensuring delivery of the service in accordance with the agreed service specification and Trust Policies.

The successful applicants will require post registration experience in acute paediatric care and have the skills and ability to work as part of a team and autonomously, making appropriate decisions to manage the care of children on the caseload. Please see personal specification for more details.

Main duties of the job

  • Plan and organise own caseload in order to meet the needs of the children and families.
  • Develop, assess, implement and evaluate plans of care working in partnership with children and families
  • Provide specialist knowledge and skilled nursing care to children and families in their own home or community setting
  • Be able to work flexibly and participate in an on call rota to provide 24 hour cover.
  • To be able to participate in an ghost on call rota that will allow us to step up to 24 hour on call cover when required (rota's are produced 6-8 weeks in advance).
  • Establish and maintain links with professionals and agencies involved in the child's care
  • Identify and report any cases of suspected child abuse
  • Assist in planning and organisation of individual care for children and young people by attending assessment, discharge planning and safeguarding meetings.
  • Caring for terminally ill children and their families at home, including out of hours cover.
  • Require skills for assessing and interpreting signs of acute illness or deterioration and take appropriate action
  • Lone Working - Having to be aware of personal safety as a result of lone visiting and working with the potential for verbal and physical aggression.
  • Work autonomously and hold a caseload as the primary nurse
