We are seeking a highly motivated and enthusiastic Band 6 Mental health practitioner (a nurse, occupational therapist, or social worker) to join our team in working as a key member of our early intervention in psychosis team. You will play a vital role in the delivery of high-quality care and support to individuals experiencing a first episode of psychosis. In this role you will be responsible for delivering evidence-based interventions, following a compassionate and person-centred approach that also draws on principles of assertive outreach. You will also work closely with other members of the multi-disciplinary team to ensure patients receive the best possible care and treatment.

Main duties of the job

Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.

Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.

Administer medication if appropriate to the role, complying with relevant Trust procedures and NMC Guidance.

Assess patient's individual care needs, develop, implement and evaluate programmes of care.

Deliver a range of interventions in accordance with the agreed plan of care and Team guidance including; pharmacological and psychologically informed interventions.

Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.

This post will predominantly be covering the East of the patch, which includes Burton, Tamworth, Lichfield and surrounding areas. However the team as a whole provides input to people across South Staffordshire, working out of team bases across the Trust, as well as peoples homes, GP surgeries, other public settings, and inpatient settings. Therefore, the successful applicant must be willing to travel across the geographical area where required.
