Community Mental Health Nurse (Band 5)
We have two positions available: 1 post is 37.5 hours per week and 1 post is 35 hours per week
We are advertising two posts for Community Mental Health Nurses to work within our Assessment and Diagnosis team. One post will support the 'front door' to Assessment and Diagnosis, working with the team to manage new referrals, providing community-based support to those who have increased needs in a community setting; and one post will be to support patients who are in the post diagnostic period, as clinically indicated.
This is an exciting opportunity to join a dedicated team who ensure the service user, and their family is truly at the centre of everything we do; and we would welcome applications from both experienced and newly qualified nurses. We are looking for caring, compassionate and highly motivated individuals who have a specialist interest in dementia. You will need to have excellent communication skills, be enthusiastic and flexible in your approach, and demonstrate an understanding of the complexities involved in this specialist area.
Main duties of the job
- Undertake nursing assessments and deliver interventions to service users who are waiting for a formal diagnostic appointment
- Support the team in the management of new referrals
- Liaise with referrers, service users and their families
- Liaise with internal and external teams
- Support those in post-diagnostic period where there is a specific need and work in conjunction with our Specialist Dementia Nurse Practitioners
- Involved in service development
