Job summary

An exciting opportunity has arisen for a committed, enthusiastic and versatile individual to work within the Unplanned Care, D2A team within the Stafford area. This fast-paced role will promote and support the discharge of individuals out of hospital and provide daily multi -disciplinary working, with a range of professionals in the social care, health, community and private sector. The service predominantly supports individuals over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

Main duties of the job

For this role, it would be expected that you have knowledge around completing evidence-based assessments in line with The Care Act 2014, Mental Capacity Assessments and have some knowledge of Safeguarding Vulnerable Adults. Working in a supportive team, you would be expected to complete assessments within agreed timescales to meet the increased demand of discharges from the acute hospitals and be flexible to your working role. You will be predominantly working within the community, completing visits and be involved with regular multi-disciplinary team meetings to promote and advocate for the individuals and their families and carers.   • Level 3 Diploma in Adult Care (Regulated Qualifications Framework) or equivalent, plus additional knowledge and experience of care practices and procedures to diploma level.
