Job summary

We are offering an exciting opportunity to join our Integrated Care Coordination for Urgent Care Team as a Band 6 Clinical Triage Nurse / Paramedic based in Hanley, Stoke on Trent.

You will either be a NMC registered Triage Nurse or HCPC registered Paramedic and you will provide triage assessment, clinical support, assistance and professional expertise to all health and social care registered professionals requiring assistance and support with Admission Avoidance.

The Service is the access point to take referrals from professionals requiring Health or Social Care Service and you will work as a key member of the Integrated Care Coordination Team to ensure professionals are able to make referrals through one access point andwork independently, utilising assessment and evaluation skills to full ability in order to avoid inappropriate hospital admissions, adjust care programmes as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patient's best interests.

Due to the nature of this role, you will be required to work on a shift Rota basis over 365 days, covering weekends and out of hours working.

Main duties of the job

As Clinical Triage Nurse / Paramedic you will work in conjunction with the Clinical Team Lead, you will offer clinical triage and support where appropriate. This may range from signposting the referral caller to the appropriate service, making a direct referral to another service or offering immediate clinical advice or treatment.

As part of this role, you will also be expected to:

  • Provide high quality, clinical triage using knowledge, skills and critical thinking and professional judgement.
  • Control and manage calls effectively by using skilled questioning, active listening and excellent verbal communication skills in a calm, professional manner.
  • Offer sound clinical advice.
  • Have the ability to prioritise tasks, cases and situations appropriately to ensure effective delivery during periods of high demand.
  • Work independently, exercising assessment and evaluation skills to full ability in order to avoid inappropriate hospital admissions, adjust care programmes as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patient's best interests.
