An opportunity has arisen to recruit a band 7 Mental Health Nurse Practitioner within the Older Adults Outreach Team. This post is required due to capacity and demands on the service and longer term workforce strategy in order to continue to meet the requirements of the service.

Applicants should possess the knowledge and skills to enable them to work in the older adults and dementia speciality. This includes working with challenging and complex needs of Older Adults. Applicants must have their non - medical prescribing qualification.

Applicants will be required to work closely with the existing multi-disciplinary team members. The role will include close working with physical and social care colleagues along with our GP and primary care colleagues.

Main duties of the job

  • Delivery of a high standard of nursing care to individuals referred to the Older Adults Outreach Team
  • Provide a specialist mental health nursing and medical assessment/ reviews,
  • To work in partnership with fellow team members and internal/external partners.
  • To undertake their own professional development and compliance with mandatory training.

  • Evidence of professional updating and development within mental health services for older people
  • Non-medical prescriber

Desirable

  • Diploma equivalent qualification in sphere of professional practice

Experience

Essential

  • Experience of delivering therapeutic interventions to clients with dementia and their carers
  • Experience of working with a range of statutory, non-statutory agencies
  • Experience of working with more complex and challenging needs in older adult care
  • Experience of user and carer involvement working

Desirable

  • Experience of working therapeutically in community settings
  • Minimum of one year's post registration experience working in a mental health setting
  • Experience of supervising junior staff
