Job summary
We are excited to offer an opportunity for a NMC Registered Band 5 Nurse to join our District Nursing Team in East Staffordshire.
We are an innovative, integrated Community Service covering the East Staffordshire area including the Burton Town area, Uttoxeter and the surrounding Villages.
Our team provides care to patients in their own homes, covering care homes and we work closely across the Nursing teams, AHP's including Podiatry, Physio, Falls Team, Stroke and Rehab team including our out of hours service, to ensure that our patients are at the centre of the care provided. We collaboratively deliver our care across all the professions, recognising the importance for patient centred approach, working together across all the nursing and therapy teams
Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.

Main duties of the job
As a Community Staff Nurse, you will work independently to deliver a high-quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions.
As part of this role, you will be expected to:
- Provide wound care, palliative and end of life care, leg ulcer management, continence and catheter care, management and administration of injections such as insulin and B12.
- Plan, implement and evaluate programmes of care, adhering to policies and procedures.
- You will work independently in a community setting, participating in the ongoing assessments.
- Maintain sensitivity at all times to the emotional needs of the service user and their carers offering reassurance and empathy. This may be in regard to the nature/prognosis of their condition and implications of the same.
- Work with individuals and carers who may find it difficult to engage with the service or aspects of the service.
This is a full-time opportunity, but it may be possible to offer a part-time role. You will be required to work on a rota basis over a 7-day working week.
