District Nursing Sister/Charge Nurse - Band 6

Job summary

We are offering an exciting role for a NMC Registered Band 6 District Charge Nurse to join our Stone and Eccleshall PCN based at Trentside Clinic, Stone.

We are a friendly and innovative District Nursing team covering GP practices in the Stone and Eccleshall locality. As a team we have a great interest in moving District Nursing forward in line with local population and service needs.

You will hold a key role in building and maintaining excellent working practices with Social Care and Allied Health colleagues within the primary care network and are looking for someone with excellent clinical, communication, interpersonal and team skills to work collaboratively across all care settings.

You will take responsibility for the planning and delivery of care and the management of staff in the absence of the Operational Lead and will demonstrate managerial and leadership skills and be able to make sound clinical decisions independently.

You will hold a Specialist Practitioner Qualification in District Nursing or currently working towards the qualification, Post Graduate studies in Nursing and Mentorship or Assessor Qualification.

We would also consider a Senior Staff Nurse if they have undertaken an accredited University Module relevant to the role and who have significant experience working in a community setting.

Due to the nature of this role, you will be expected to travel over a large geographical area and will ideally have a valid driving license and access to a car.

Main duties of the job

As District Charge Nurse you will manage a caseload over a wide geographical area by providing support and supervision to staff to enable provision of high-quality person-centred care to patients within their own homes and clinic settings.

As part of this role, you will be expected to:

  • Be skilled in the assessment of complex/specialist care/support needs and in the development, implementation and evaluation of care/support
  • Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service developments/ improvements
  • Work with the Operational Lead undertaking delegated tasks in team management
  • Demonstrate initiative and be creative in finding solutions
  • Experience of:
    • Palliative care
    • Management of continence
    • Administration of medication
    • Management of long-term conditions
