Job summary

A rare and exciting opportunity has arisen for an enthusiastic and self-motivated individual to join the South Staffordshire Hospital Discharge Team based at Great Wyrley Health Centre.

The role of the Discharge Liaison Sister / Charge Nurse will be based in our Triage hub, providing clinical expertise to our referring acute hospital trusts. The main referring hospitals in the South of the county are Russell's Hall Hospital, Newcross Hospital, Walsall Manor Hospital, Good Hope Hospital and Queens Hospital Burton.

Main duties of the job

The successful candidate will be part of a triage team that receive referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients' needs as part of a multi-disciplinary team with a key role in decision making. Part of the role of the Discharge Liaison Sister / Charge Nurse will be to support end of life patients to receive the best quality care in the place of choice determined by the patient.

The post holder will liaise with a variety of people (service users, colleagues, member of the acute hospital team, families/carers and other professionals) to provide an accurate assessment of an individual's ability/potential for reablement via the discharge to assess pathways alongside supporting admission avoidance assessments to prevent a hospital admission.
